Dr. Reza was born in Iran and obtained his Doctor of Dental Surgery degree from a university in Tehran in 1999. He started working in a public hospital for about two years. Afterwards, he ran three of his own private dental clinics in Tehran. He performed all of the dental procedures, specifically static dentistry, for many patients of all ages, with zero records of complaints and for more than 12 years. He then came to Canada in 2013 and passed all of the exams and courses of the National Dental Examining Board of Canada with success. From the time he has arrived, he has studied and worked hard to reach his current level of success and to become a member of the Ontario Dental Association, the Canadian Dental Association and the Royal College of Dental Surgeons of Ontario. Dr. Mozhgan graduated from dental school in Iran in 2004. After working for two years in a multi-disciplinary clinic, she opened her own practice which she ran until 2013 when she immigrated to Canada. After relocating to Canada, she passed the National Dental Examining Board of Canada exam and registered with the Royal College of Dental Surgeons of Ontario in 2018. Dr. Mozhgan is a standing member of the RDCSO and of the Canadian Dental Association since 2018. Dr. Mozhgan believes in lifelong learning and as such has taken many courses along the way to increase her scope of practice so that she may implement her expertise into her work. Dr. Mozhgan enjoys being an integral part of her community. When she is not working, you can expect to find her running, hiking and enjoying the beautiful nature that Canada has to offer with her family and her twins whom she adores.

Do you have to reapply for CDCP every year?
You must renew your CDCP coverage every year. The program is tied to your latest tax return and income information. If you complete the renewal during the official window, you can use a simpler process — but if you miss it, you must reapply. This yearly check allows the federal government to confirm that households still meet income limits and do not have existing dental coverage through private insurance, an insurance or benefits company, or health and wellness accounts.
How do I renew my CDCP?
The most common way to renew CDCP online is through My Service Canada Account during the renewal window. File your latest income tax return first so the system can use up-to-date income information. If you are uncomfortable online, call Service Canada at 1-833-537-4342. Renewal options only appear while coverage is still active — if the system shows no active CDCP coverage, you likely need to re-apply.
How do I check if my CDCP is still active?
Log in to your My Service Canada Account and view the Canadian Dental Care Plan section. It lists whether coverage is active, pending, or ended and shows benefit year dates. You can also check the Sun Life member portal. Ask the front desk at Dentistry on Guelph to verify coverage electronically when you book.
What if I missed the June 1 CDCP renewal deadline?
If you missed the June 1, 2026 CDCP renewal deadline, you cannot renew that benefit year and must re-apply as a new applicant. Your previous coverage ended June 30, 2026, and there will be a coverage gap until your new application is approved. Re-apply as soon as your 2025 tax return is filed to shorten the gap.
When does CDCP reset?
CDCP resets at the start of each new benefit year when new annual limits begin. For many enrollees, the 2026–2027 benefit year runs from July 1, 2026 to June 30, 2027. Renewal windows usually open several weeks before the benefit year ends. Confirm your own dates in MSCA or Sun Life and plan dental appointments accordingly.
Can I have CDCP and other dental insurance at the same time?
Under current rules, people with existing dental coverage — including private dental benefits, employer dental insurance, pension benefits, wellness accounts, or a group plan — are generally not eligible for CDCP. If you gain new dental coverage during the benefit year, update your information with Service Canada through MSCA. CDCP is designed for eligible Canadians without other dental benefits and is not a program that replaces benefits you already have.
What should I bring to my first CDCP appointment?
Bring your Sun Life CDCP member ID number, a copy of your determination letter or digital card, and government-issued photo ID. It also helps to bring a list of medications, recent dental concerns, and previous dental records if you are new to Dentistry on Guelph.
